This document consolidates the reporting requirements under Sections 5-5 and 5-5.8 of the Illinois Public Aid Code (305 ILCS 5/), Section of the Disabilities Act of 2003 (20 ILCS 2407/) and Section of the Children's Health Act (215 ILCS 106/).
